Dose, volume, and units are three views of one number

A dose is an amount of peptide, usually written in micrograms. A syringe cannot measure micrograms; it measures volume. So a dose only becomes drawable once the concentration of the vial is known, and the concentration depends entirely on how much bacteriostatic water was added.

This is why the same 250 mcg dose is a different mark on the syringe for two people holding identical vials. The dose has not changed. The dilution has.

Converting between mcg and mg

One milligram is one thousand micrograms. Doses are commonly written in mcg while vials are labelled in mg, so the conversion sits in the middle of nearly every peptide calculation and is a common place for an error to enter.

How many doses a vial holds

Divide the vial strength by the dose, with both in the same unit. A 5 mg vial against a 250 mcg dose is 5 mg ÷ 0.25 mg, which is 20 doses. The diluent volume does not change this. It changes how much liquid each of those 20 doses occupies, not how many there are.

How do I convert a peptide dose in mcg to syringe units?

Divide the mcg by 1,000 to get mg, divide that by the vial concentration in mg/mL to get millilitres, then multiply by 100 for units on a U-100 syringe.

How many mcg are in a mg?

One thousand. So 500 mcg is 0.5 mg, and 2,000 mcg is 2 mg.

How many doses will my vial give me?

Vial strength divided by dose, with both expressed in the same unit. A 10 mg vial against a 500 mcg (0.5 mg) dose gives 20 doses. How much water was added does not change the count.

Why do two people with the same vial draw to different marks?

Because they added different amounts of bacteriostatic water. Dilution sets the concentration, and concentration sets the volume that carries a given dose.

Can I change the dilution partway through a vial?

Adding more diluent to a partly used vial changes the concentration of everything remaining, so every subsequent calculation would need to be redone against the new figure. Any change like this is a conversation for your healthcare professional.
